WSMR STANDARDS:

• 100% of requests are processed. Due to the detailed instructions and background check that has to be conducted on the packets, no standard time frame is prescribed.

SUPPLIER STANDARDS:

• Ensure requests are properly completed.

PROCESS:

• Soldiers can pickup forms from Bldg 465, Personnel Services Branch.
• S1 Kit consists of the following required forms:
• Form N-426 (Request for Certification of Military or Naval Service)
• Form G-3325B (Biographic Information)
• Form N-400 (Application of Naturalization)
• Forms 86 (1) Authorization for the Release of Information and (2) Medical Information Release
• Following receipt of a background check and upon completion of all forms, soldiers must provide the following documentation:
• Alien Resident Card
• Two passport photos
• AG sends completed S1 kit to Nebraska for processing.
